Lengthy, paper-based forklift certificates are too messy to carry on a daily basis. Make it easy for forklift operators to store their certification on site. Wallet-sized Forklift Certification Cards are handy and a proof of completion of their training. These wallet cards are an affordable solution to avoid OSHA fines too.
- Forklift Training Cards include blank fields for driver’s name, authorization date, and authorized trainer’s signature, plus a safety checklist on the back. All the hand-written information is protected with a self-laminating flap.
- Easily write on the rigid, 30 mil thick plastic cards using a permanent pen or marker. Self-laminating cards are waterproof. They can withstand oil and grease too.

Whatever works...
I truly like the cards, but it's too hard to write name, date and sign for instructor unless you have a special extreme fine tip marker. Those areas need a spot that you can write with a pen on it. I had to use a label maker to affix that information.
